Please use this form to request CEUs from the Brethren Academy for Ministerial Leadership for webinar attendance. Your request for CEUs will be verified with your attendance at live online training sessions. A CEU certificate will be sent to you by e-mail after processing.

• Please submit all requested information, and complete a separate form for each individual or CEU request.
• CEU credit is currently available for those participating in the live broadcasts only, not those viewing a recording later.
• Please submit this form within a day or two of attending the webcast, so that we can efficiently process all CEU requests in a batch.
