Guidelines for gathering
"Turning Points" information
Names—Always use first and last name of each individual (i.e., John Smith, Mary Smith)
New Members—(1) the first and last names of each new member, and (2) the name of the congregation joined.
Wedding Anniversaries—(1) the full names of the couple, (2) the town and state where they reside, and (3) the number of years they have been married if one of the following: 50, 55, 60, 65, 70, and any anniversaries beyond 70.
Deaths—(1) the full name of the deceased, (2) the deceased's age, (3) the deceased's home town and state, and (4) the date of death.
Must be submitted in writing—Please type or print legibly
