• This curriculum order form is actually a packet of forms. You may need the entire packet, or you may need only one page.
  
• If you are sending more than one sheet, fill out the full customer information on only the first sheet. On subsequent sheets, write in the church code and name (to ensure that orders don’t get separated by mistake).

Annual orders:

• For convenience, you may place orders for all four quarters before the curriculum year starts. This annual order should be placed early in the summer, since we begin shipping fall curriculum late July. Annual orders should be mailed, faxed or emailed. A new annual order must be placed in the fall of each year.
  
• You can change orders for upcoming quarters by calling Brethren Press at least two months before the quarter begins.
  
• The Seed Packet is a quarterly newsletter on topics of interest to those involved in Christian Education. It is published by the General Board’s Brethren Press and Congregational Life Teams. You will automatically receive one copy with your curriculum order. If you would like additional copies, please indicate the quantity needed and we will include them with your curriculum each quarter. _________________ additional copies needed.

Order early: We begin shipping materials 6 weeks before the quarter begins.

How to return curriculum:

• Curriculum must be returned within 30 days of the start of the current quarter.
  
• Include Need to Send it Back form included with shipment. Include a copy of the original invoice or packing slip. If these are not available, include a list of the items being returned. Be sure to include the name and address of your church.
  
• State the reason for the return.
