Please fill out this form completely, print it out, sign it, and hand it in to your group leader. Note: the parent or legal guardian of any youth under age 18 must also sign.

BDM Form Authorization of Consent to Emergency Care of Minor (6 KB)
The parent or legal guardian of any youth under 18 years of age is required to sign this form in order for the youth to participate on a Brethren Disaster Ministries project.
